Thunder Bay Counselling Centre’s Youth-in-Transition Worker Program supports young people between the ages of 16-24 as they leave the care of the child protection system, and become self-sufficient adults. The Program facilitates access to support services that help individuals build resilience as they exit care, and enhance their quality of life as they enter adulthood.
Youth-in-Transition Workers work one-on-one with individuals to:
- Access health and mental health services
- Secure stable housing
- Obtain education and training information
- Connect to employment services
- Identify and access life skills support
- Locate legal services

Dilico has a Youth Transition Worker who works with young people between the ages of 16 to 24 as they leave the care of child protection services.

Ontario Native Women's Association
The Youth in Transition Worker (YITW) program is to support eligible Aboriginal youth in their successful transition out of the child welfare system into adulthood. The YITW will also support youth to connect to existing supports and resources within their communities including housing supports, education resources, employment services and training, life skills training (e.g. financial management, household management), health and mental health services, and legal services.
